This presentation will recall three tools for analysis of events or process failure leading to risk of liability losses in healthcare, identify proactive and reactive use of RCA, ACA, and FMEA for prevention and risk mitigation, and assess which tool is best to use, based on scenarios involving examples of actual healthcare events.

The presentation will discuss the nature of risks associated with protected health information, identify how to mitigate risk, and take a deeper dive into specific strategies and issues.
